The supernumerary teeth are anomalies of the dental development related to the number that can be or not associated to some syndrome. The objective is to report an unusual case of a twopeaked paramolar in the superior maxillary.
Due to pain episodes, a 20 year-old patient was presented to be assisted at the Dental Clinic of the Del Valle University, La Paz - Bolivia. To the dentistry exam it was evidenced the presence of a two-peaked tooth in the vestibular region of the left superior molar area, the pericapical conventional x-ray was not of utility, for that reason a 3D Cone beam study was requested to confirm the diagnosis.
The study 3D Cone beam confirmed the presence of a paramolar and the absence of the second left superior premolar germ; therefore, the supernumerary piece extraction was carried out. Considering that it is not very probable the presentation of this type of cases, as they are not very unusual, is important that the doctor and the odontologist were familiarized with the supernumerary teeth to be able to diagnose and to prevent possible complications.
