SEVERE PESTICIDE POISONING BY ALUMINIUM PHOSPHIDE FUMIGANT TYPE PESTICIDE SANTA BARBARA HOSPITAL, 2014 A CASE REPORT

Acute aluminum phosphide poisoning is lethal and its toxic component is a gas called phosphine. This substance is a pesticide used to conserve grain crop, which is highly toxic to insects in all its stages.
Voluntary ingestion is a suicidal intent. In humans, death occurs because of a deep state of shock, myocarditis, lethal arrhythmias and multiple organ failure. The lethal dose of the poison range goes from 50mg to 500 mg. Even a Iower dosage can provoke death. The mortality rate reported in the literature varies between 40% and 80%, has conditioned the highest number of deaths compared to all the poisons used. This poison has usually been used as a method of suicide attempt in adolescents and young people in rural areas. For its high lethality is imperative to know their clinical presentation.
The case reported a 15 years old female patient from the municipality of Yamparaez with a history of intentional ingestion of pesticides, coming to the emergency room one hour after ingestion, presenting altered state of consciousness when admission occurred, with frank respiratory failure, hypotensive, tachycardia, tachypnea. After completion of all the measures for decontamination and hemodynamic support for 9 hours, the patient died.
