The presence of pulmonary hypertension (PH) in pregnancy is rare and constitutes a greater challenge given the severity of the pre-existing cardiopulmonary disease together with the changes typical of pregnancy, which normally aggravates the underlying disease, frequently being associated with maternal and fetal morbidity and mortality. Presentation of the case A 39-year-old female patient in second digestion, with a history of pneumonectomy of the left lung 23 years ago, went to the Emergency Department due to regular uterine activity. Pneumology indicates that this is a patient with a picture of severe obstructive pulmonary syndrome with a recommendation to terminate the pregnancy through the abdomen. The surgical procedure was performed complying with the respective recommendations. Once the procedure was completed, the patient went to the Intensive Care Unit for hemodynamic control. In all cases of severe pulmonary hypertension, multidisciplinary management must be carried out to prevent hemodynamic changes from being minimal
