El conocimiento sobre cómo manejar a un paciente traumatizado se vuelve
un requisito obligatorio para todo el personal de salud que trabaja en un
servicio de emergencias. Un ejemplo de esto, es que este tipo de evento
es la principal causa de muerte en adultos jóvenes en los Estados Unidos,
representando el 10 % de todas las muertes (1). La mayoría de estas
muertes ocurren en el lugar del accidente o en las primeras 4 horas después
de acudir a una sala de emergencias (2). Las lesiones presentadas pueden
variar en relación directa con el mecanismo del trauma.
En el momento de ingreso del paciente, se debe realizar la evaluación
primaria, que se lleva a cabo mediante las siglas ABCDE (3):
• Airway (Vía aérea): Evaluación y protección de la vía aérea junto
con la estabilización de la columna cervical.
• Breathing (Respiración): Evaluación de la mecánica ventilatoria.
• Circulation (Circulación): Control de hemorragias y perfusión
orgánica.
• Disability (Incapacidad): Examen neurológico.
• Exposure (Exposición): Exposición del paciente, seguido de
cuidados contra la hipotermia.
Cada uno de estos puntos presenta hallazgos que pueden suponer un riesgo
para la vida, y se realizará el tratamiento inmediato de los mismos (por
ejemplo, obstrucción de la vía aérea, neumotórax a tensión).
A. Es importante destacar la protección de la vía aérea, por lo que se
recomienda realizar la secuencia de intubación de rápida. Esta técnica
implica la preoxigenación del paciente, seguido del uso de un fármaco de
inducción (Propofol, ketamina, etomidato) y de un relajante neuromuscular
(succinilcolina, rocuronio). El uso de esta técnica ha demostrado un 90%
de éxito en el primer intento (4). También se sugiere utilizar la técnica del
3-3-2 para valorar rápidamente la vía aérea.
B. Valorar la mecánica ventilatoria y la oxigenación del paciente a través
del oxímetro de pulso, buscar la presencia de lesiones en la pared costal,
alteración o ausencia de los ruidos respiratorios a la auscultación; en caso
de un neumotórax, se realizará la descompresión mediante toracostomía
con aguja en el quinto espacio intercostal a la altura de la línea media axilar
(5). En presencia de un paciente inestable debido a hemotórax, se opta por
el uso de un tubo de tórax (Francés), Nro. 24 al 28 (3).
C. La hemorragia se presenta como la principal causa de muerte prevenible
en pacientes traumatizados, llevándolos al estado de shock, ya que no
